The NCCD Guidelines ask for evidence in four general areas. Superadjust names them Need, Consultation, Adjustments and Monitoring, and suggests one or two from the words in your note. The first is the main one. Change them before you log if you see it differently.

Questions about NCCD evidence logging
Log what you did for the student, why, and how they responded. The NCCD Guidelines ask for evidence in four general areas: the student's assessed needs, consultation with the student and their family, the adjustments provided, and ongoing monitoring and review. Superadjust calls these Need, Consultation, Adjustments and Monitoring, and suggests the pillar for each entry. The guide to the four areas of evidence goes deeper.
The NCCD sets no number of entries. In the Guidelines' words, a student needs evidence of adjustments for at least 10 weeks of school education in the 12 months before Census Day, and the weeks can be non-consecutive, except for Extensive adjustments. Log when you make an adjustment, and set regular ones to repeat. More on the NCCD 10-week rule.
No. Schools are not required to create new or additional evidence for the NCCD. Evidence is sighted in the form it already exists. A short, dated entry logged at the time is a record of work you were already doing.
No. Under the NCCD, a disability can be diagnosed or imputed: judged to exist on reasonable grounds and evidence, after consulting the student or their associate. Log what you do. The level of adjustment and the category are school team decisions. More on imputed disability.
